How should I come out? Who too? And when?

I would suggest you talk to your boyfriend and those friends who you've came out to. I'm sure they would have some suggestions, considering you all live in the same community(I'm assuming). Alternatively, you could tell your parents before going to college and give them some time to think it over. Good luck.
